Transitional and Fitness Programs
Transitional Fitness is for patients who have been discharged from physical therapy, but require additional therapy to maintain long-term goals.
Cross Keys Physical Therapy established this program in 1991 to allow patients a transitional period to adjust from a fully supervised therapy program to total independence. The patients referred to this program are in need of continued care, but no longer require one-to-one guidance. They are able to perform their rehabilitative exercise routine independently; however, they are not ready for total independence. This program offers the opportunity to continue with their rehabilitative program in a structured environment with the guidance of trained professionals.
The therapy staff is available to discuss the patients concerns and for instruction in new techniques or modifications in the patients current program. Independence is encouraged/required so that the patient begins to take responsibility for their care and takes precautions to prevent re-injury.
Most importantly, this program provides a patient the opportunity to be re-evaluated on a quarterly basis. This evaluation will document the individual’s progress in the program and/or the problems they have encountered.
This program is available in ALL of our facilities.
